Palms Medical Group offers a variety of services to meet your needs including Behavioral Health Care, Chiropractic, Dentistry, Family Medicine, Pediatrics, Prenatal, Pharmacy, and Xrays. We also offer a Graduate Medical Program. We have locations throughout North Central Florida.
Palms Medical Group provides comprehensive affordable services to persons of all ages with and without insurance. For adults, we have Family Medicine and Internal Medicine physicians, Nurse Practitioners and Physician Assistants who are well educated in preventive health care and capable of treating both acute and chronic medical conditions.
Comprehensive care for adults includes the following:
- Health screenings
- Annual health assessments, including well-woman examinations
- Adult immunizations
- Chronic disease management: high blood pressure, diabetes, high cholesterol, heart disease, COPD, asthma
- Sexually transmitted diseases diagnosis and treatment
- Nutrition counseling
- Vision and Hearing Screenings
- Acute medical illnesses: bladder infections, bronchitis, pneumonia, minor orthopedic injuries
- 24-hour on-call coverage

Palms Medical Group provides pediatric care at all locations, while Palms Pediatrics is dedicated to pediatric care. In addition to the diagnosis and treatment of illnesses and minor injuries, Palms Pediatrics offers:
- Childhood immunizations
- Free required childhood immunizations
- School and sports physicals
- Hearing and vision testing
- Well-baby exams
- ADD/ADHD evaluations

Palms Medical Group provides dental care at Palms Dentistry in Lake City, Trenton and Williston, Florida. Our Lake City office serves patients from age 3 to adults. Our Trenton and Williston offices serve patients aged 18 months (with their first erupted tooth) to 20.
In Lake City: We currently do not accept any commercial or private insurances. We accept Medicaid HMO’s. Please note that Dental Medicaid insurances have frequency limitations on their covered services that you may be responsible for at the time of your visit. We also offer a sliding fee program. We also offer same day appointments. Contact us today to schedule an appointment.
In Trenton / Williston: Palms Dentistry of Trenton accepts all Medicaid HMO’s, Argus and MCNA through the Florida Healthy Kids Plan, and any commercial PPO’s with out-of-network benefits except for Delta Dental. Please note we charge a $10 processing fee for commercial insurances. We also offer a sliding fee program and other financial assistance options.

- Comprehensive Oral Exams
- Limited Oral Exams
- Periodic Oral Exams
- Fillings – one or more surfaces
- Dental Cleanings – adult and children
- Sealants
- Extractions – simple, surgical and wisdom (some referrals to oral surgeons may be necessary)
Palms Medical Group is pleased to share the expansion of our Behavioral Healthcare Program.
We have added two full-time and two part-time Licensed Clinical Social Workers (LCSWs) to our Palm's staff who provide counseling services to children and adults in select clinics within our service area.
Our full-time LCSWs devote 100% of their scheduled time to the identified counseling, assessment, and screening needs of PMG patients and consultation needs of PMG staff/Providers.
Our PMG part-time LCSWs work in conjunction with the Graduate Medical Education Program (GME) through area Hospital Corporation of America (HCA) facilities to provide Behavioral Health training to medical residents in the Family Medicine Program while providing some crisis intervention services for PMG. Palms also continues to work in partnership with Meridian Behavioral Healthcare to provide counseling and psychiatric services for adults in some of our sites.

Palms Pharmacy offers services for new, refill and transfer prescriptions as well as commonly used over-the-counter drugs.
Conveniently located in some of the Palms Medical Group offices, our pharmacists and pharmacy technicians ensure correct drug selection and dosing and educate clients on proper usage of medications.
Palms Pharmacy is available to all Palms patients and is competitively priced with additional discounts and special programs being available through the 340b drug program. The 340b drug program is exclusive to Palms patients.

Palms Complementary Alternative Health offers preventive care and progressive methods to treat stress and illness, including chiropractic & massage therapy.

Palms Medical Group provides prenatal care for maternity patients in the most effective way for our community. By partnering with midwives who deliver at UF Health Shands Hospital, our patients receive individualized care during their pregnancy based on their pregnancy complications.
Palms Medical Group is pleased to participate in the training of medical residents in our Gainesville, Trenton and Orange Park offices. Residents are individuals who have graduated from medical school and are gaining extensive training and experience in either family practice or internal medicine.
