Palms Medical Group opened its doors for the first time in 1971, serving the community and providing care to underinsured and uninsured patients. We were officially designated as a Federally Qualified Healthcare Center (FQHC) in 1988, enabling us to receive grants for community health services. Over the years, Palms Medical Group has expanded in size and capability. This rich history has been interwoven with quality care to a diverse population of community patients.
Now, with offices in various North Florida communities, same-day scheduling, flexible hours, and bilingual staff, Palms Medical Group gives patients the care they need, when and where they need it. We are committed to providing quality, accessible care for everyone in our communities.
Expanding on standard primary and preventative care, Palms offers pediatric care, behavioral health, and dental services, as well as complementary alternative medicine, such as chiropractic care. Also, several of our locations have labs, X-ray equipment, and a pharmacy, all under one roof.
As an FQHC, Palms accepts most public and private insurance, including Medicare and Medicaid. Palms offers financial assistance in the form of a sliding fee scale, based on income. We will not deny a patient medical care because of lack of insurance or income.
Palms Medical Group operates as a Patient-Centered Medical Home (PCMH), always placing patients at the center of their care and continuously building upon a bedrock of quality relationships between them and their clinical care teams.
Palms Medical Group employs a variety of health care providers, from Medical Doctors (MD) and Doctors of Osteopathic Medicine (DO), to Physicians Assistants (PA-C) and Advanced Practice Registered Nurses (APRN). Regardless of the letters behind their name, every one of our providers is equally dedicated to our patients, our mission, and our communities.
Our mission is to improve the health and well-being of our community by providing accessible, compassionate, and affordable healthcare services. We envision a future where every member of our community has access to the resources they need to achieve optimal health.
Our outreach programs and partnerships with local organizations help us to reach underserved populations and provide education and resources that promote healthy living. A few of these outreach programs are as follows:
